Combination Code Request Procedure
The Combination Code Request form found below, is used for NNMC Staff and Faculty to be able to request combination codes for offices and classrooms and areas that allow it. Please fill out the form completely, this includes the Keyholder; Position; Department; Banner I.D.; Home address; City; State; Zip; Work Phone; Home Phone; Work Status; and Employee Signature. The form must then be approved by Chair/Director/Supervisor and Dean/Provost. Once the approval is obtained, the original form can be turned in to the Facilities Department. Combination Codes will be issued and a member of the Facilities Department will contact you via phone or email to issue you a combination code.
It is extremely important that Faculty and Staff do not share their Combination Codes with other Students, Faculty, and Staff.
